Leaving the hospital and can't go home: what happens next
It’s one of the most stressful calls a family gets: Mom or Dad is ready to leave the hospital or rehab, but the care team says it isn’t safe to go home. Often you have only a few days.
First, take a breath and ask questions
Talk with the discharge planner or case manager. Useful things to ask:
- What level of care do they recommend: assisted living, memory care, an adult family home, or more skilled care?
- What’s the expected discharge date?
- Will a short rehab stay come first?
- Are there therapy or equipment needs the next place must handle?
Then make a call
This is exactly when a local advisor helps most. I’ll talk with you about what’s needed, look for openings that fit your loved one’s care needs and budget, and help you tour quickly, in person or on your behalf if you’re out of town.
You don’t have to make this decision alone, and it doesn’t cost you anything. Call me at 509-412-9123 as soon as you know a discharge is coming.
